Our goal is to provide excellent, empathetic care that keeps you and your family in the best health. The Transitions of Care Clinic provides follow-up care for patients who don’t have a primary care physician and who recently had either an inpatient hospital stay or emergency visit. We serve as a convenient alternative to the emergency department.
